The flood of male enhancement products came about with the deregulation of the natural products industry and the concurrent introduction of a drug known as Viagra. The accidental discovery of a side-effect produced by this heart medication undergoing clinical studies gave birth to a new class of prescription drugs and spawned a renewed interest in herbal aphrodisiacs as an exploitable market. Aphrodisiacs have a long history and most of it is myth - fantastic stories with little or no basis in fact. As the pharmaceuticals companies scrambled to reap the profits to be made from new treatments for erectile dysfunction, natural products companies began to emerge with fantastic claims and equally over-priced products.
FACT 1 - No pill, patch, gel or liquid can make your erection longer. Claims to increase length 2 to 3 inches are always referring to flaccid length not erect length. Anyone that claims to increase the length of your erection 2 to 3 inches is flat out lying. So why bother? Some formulas include ingredients that will improve erection performance as well as overall girth to some degree. The demand for ED products has fueled a great deal of botanical research. There is some scientific evidence that certain plants aid in vasodilation and testosterone production, for example. There is also clear evidence however that the sources of some exotic herbs such as Tongkat Ali, for example, have been tainted by toxic substances such as mercury.
FACT 2 - No mechanical device (pump or extender) has been shown to produce a permanent change in erection length. There is evidence that use of a traction device or extender can increase the erection length marginally, (see below) however no studies have been done to test whether these changes are, in fact, permanent.
FACT 3 - Phalloplasty (plastic surgery of the penis) is the only method of increasing erection length. This is accomplished by cutting the suspensory ligament. Extenders are usually used post-operatively to improve results during healing.
Male enhancement used to mean "natural male enhancement" by default. Many sites now add confusion by including prescription products under this heading. Transdermal hormone replacement therapies are sometimes prescribed to restore libido when there is a marked drop in testosterone.
For most men, the bottom line is performance. Long-term nicotine, alcohol, and caffeine abuse can all lead to depletion of essential nutrients and minerals. Lack of exercise means reduced energy and limited performance. Raise your performance and confidence at the same time by addressing these issues first - exercise regularly, eat right, take daily supplements to combat the effects of aging and excess consumption of alcohol or caffeine. Watch your intake of sweets - they cause radical changes in mood and behavior.
The prostate problem usually shows up in your forties and it seems to be linked to diet as well as aging. Try taking vitamin B6 (6 mg daily) or a B-complex formula first. If you need something more try adding standardized Saw Palmetto extract. You should be getting regular exams by age 50 regardless.
ED (erectile dysfunction) shows up around the same time and for the same reasons. We don't seem to retain minerals and vitamins for some reason and supplements are often required. Men panic when their performance levels drop but for many there is a simple solution - Zinc (15 mg daily).
Zinc is one of the most common ingredients in male enhancement formulas for this very reason. Of course, you can get it much cheaper at your local drugstore. The other key ingredient, in male enhancement formulas that work, is L-Arginine. You can get L-Arginine in bulk powder form for a mere fraction of the price you pay for it in a male enhancement formula and it works just as well. All the herbal ingredients you find in propietary formulas are also readily available in bulk form.
Stress getting you down? Try Magnesium (250 - 400 mg daily). Magnesium is known to reduce absorption of some prescription medicines so you should take it 2 hours before or after (Ask your doctor if there is any reason you shouldn't take it)

These were designed to improve post-operative healing after surgical procedures, which is why you see so many doctors endorsing them. They don't do much for you if you haven't had a surgery. Take a close look at the results. One site claims an average gain of 13 mm - that's 1/2 an inch. Another site gives more telling data - the best you should expect is 1/8 of an inch gain for each inch that you had to begin with - that's 3/4 ths of an inch for the average male of 6 inches. (Actually most men are about 5.7 inches.) More clinical studies are underway and only time will tell.
Personally, I'm not willing to dish out $300 for the privilege of torturing myself for 6 months when there's no guarantee that any of these gains are permanent. Flesh is elastic. Yes it stretches - it also shrinks.
In case you're wondering about surgical alternatives, typical results are 2 inch gain in flaccid length and 0.5 - 1 inch gain in erect length. It's expensive and like all surgery - not without risks.
